An Accountable Care Organization...

is a group of doctors and other health care providers who voluntarily work together with Medicare to provide:
• High-quality service and care
• At the right time
• In the right setting

If your doctor participates in an Accountable Care Organization (ACO), he or she will work closely with other doctors and health care providers to coordinate care for those who have traditional Medicare. If you do not have a primary care doctor, or you want to switch to a doctor in an ACO, you can find one by registering with MyMedicare.gov and choosing a Primary Clinician.

Here is how an ACO will help your doctor coordinate your care:

  • Your doctors will be part of a better coordinated team
  • You may not have to fill out as many medical forms that ask for the same information
  • Each of your doctors will know about your health issues that they have treated, and they will have a more complete picture of your entire health profile by talking with your other doctors

BJC’s Accountable Care Organization is focused on achieving improved patient-centered health outcomes and cost savings by improving care coordination and disease coordination for patients; expanding evidence-based care delivery models; and developing a structure that provides sharing of cost savings, if any, to ACO participants.

BJC Accountable Care Organization is turning 10. How well do you know it?

In 2012 BJC became the first health care provider in the St. Louis area to take on the challenge of forming an accountable care organization (ACO) to take better care of seniors. Ten years later, about 38,000 patients are covered and the organization has regularly met its goals of reducing the total cost of care while improving care quality.

BJC ACO begins remote patient monitoring for patients with CHF or COPD

In February 2021, the BJC Accountable Care Organization (ACO) began remote patient monitoring technology using Bluetooth-enabled devices and tablets to manage patients after being discharged from the hospital. Patients who have CHF and/or COPD with a recent hospitalization have been receiving the devices, but plans are to expand to additional diagnoses as well as those patients without a recent hospitalization.
