Get to know your ACO Social Work Team

BJC ACO begins remote patient monitoring for patients with CHF or COPD

The ACO would like to introduce you to our five wonderful social workers.

Robin Batchelor: Robin has been a social worker for the last 19 years; with her passion and experience primarily being in the medical field. In her role, Robin focuses on helping people see their potential and providing resources and support to aid them in their journey. She also enjoys helping caregivers obtain the resources and support that they need to care for their loved ones.

Alison Lowe: Alison is unique in that she has both her nursing and social work license. Alison started her professional career as a nurse but always had a passion for social services. Alison enjoys working with patients with mental illness. She also likes to provide support and encouragement to people who may feel like they are misunderstood or who are grieving.

Arielle Kniffen: Arielle has spent most of her career in the Medical, Home Health and Hospice social work field. Arielle enjoys problem solving ways to improve patients’ lives. Researching community resources and creating resource guides that people can use to find the assistance they need are a true passion of hers.

Kathryn (Katie) Maclean: Katie has spent her professional career in social work within health-related organizations, spending five years with the National MS Society and 10 with the Alzheimer’s Association. Older adults are her favorite population to work with, especially family caregivers and those living with dementia. She gets personal satisfaction from helping others figure out ‘unsolvable’ issues.

Cathy Ammons: Cathy has spent the last 24 years in health care at BJC as a clinical social worker. During her career she has worked in a residential treatment facility for adolescent males and in the hospital as an inpatient social worker. What Cathy loves most about her job is it allows her to make the lives of others better and lessen their burdens.

Effective care coordination among individuals with chronic and complex health conditions is a key priority of the ACO, as well as encouraging patient engagement and effective self-management. Social workers are an integral part of the care coordination team by identifying and addressing barriers to effective patient care as well as knowing insurance guidelines, medical equipment, community resources, advanced directives and crisis intervention
